La fièvre Q

Résumé

Causée par la bactérie Coxiella burnetii, la fi èvre Q est une zoonose largement répandue dans le monde. Cette bactérie est intracellulaire obligatoire et peut survivre dans l’environnement. Elle possède un potentiel épidémique, mais les facteurs conduisant à l’apparition des cas groupés humains ne sont pas bien compris. La transmission est essentiellement aérogène. La persistance et la diff usion des bactéries excrétées par les ruminants domestiques infectés jouent un rôle majeur dans la transmission. Les conséquences économiques et sanitaires pour les cheptels infectés sont encore mal connues. Une démarche diagnostique des épisodes abortifs et de gestion sanitaire consensuelle a été proposée en France. Un dispositif pilote de surveillance sera prochainement mis en place. Cet article de synthèse souligne des questionnements sur cette maladie et met en perspective les actions en cours et les orientations des recherches.

Abstract

Q fever is caused by the bacterium, Coxiella burnetii, and is a zoonosis which is found throughout the world. This bacterium, obligate intracellular in nature, and can survive in the environment. It possesses the ability to cause an epidemic, but the factors which lead to the appearance of groups of human cases are poorly understood. The transmission is mainly aerosol. The persistence and the diffusion of excreted bacteria by infected domestic ruminants play a major role in transmission. The economic and health consequences for infected herds are still poorly understood. A diagnostic approach of abortive episodes and of consensual health management has been recommended in France. A pilot scheme for surveillance will soon be established. This summary paper underlines the questions raised by this disease, outlines the actions undertaken and the orientation of research.
